On just who is responsible, God or man?
The reformed spiritual worldview is that "God from all eternity did, by the most wise and holy counsel of his own will, freely and unchangeably ordain whatsoever comes to pass", and if left at that does raise valid questions such as:

That is not the God portrayed in the bible at all. God is not the Author of evil. The enemy (satan) comes to kill steal and destroy.

An alternate idea to God ordaining what would happen is to have God "see" what would come to pass at some point. Things are different depending on when God did the seeing, but they do have something in common.
What they have in common, as far as I can see, is that if God (or anyone) knows something is going to happen, has the ability to prevent it, yet chooses to not prevent it from happening, then that means that He bears (at least some) responsibility for what happens?
Depending on when God "saw what would happen", God could have either NOT created man in the first place, or ended the world between when He created Adam and Adam sinned.
As He didn't, doesn't that make God responsible for what has happened?
And to get in first, the reformed response to God not being responsible for sin and the rest of it is man's free-will.
