That's basically a disclaimer. When a large magazine writes about something like that they'd expect thousands to try it and in this case it's likely that some people with "pre-existing medical conditions" will get sick or at least worse than they were before and potentially law suits may follow. Not because the diet is so extreme but because it's specified as lasting for 21 days non-stop. People go on all sorts of diets that are less nutritious than that one including drinking juice only but its length usually isn't specified.
