    Yeah, I just got the notification for that today. Tried filling out the form, but won't work. Figures.

    Probably would have opted out, but then
    I got my identity hacked there I found out yesterday because of a purchase I made for a Facebook award. An offer for a blood pressure watch. Was only $20. Got the watch then 2 days later my SS Debit card had 2 purchases for $138 on the 22nd & 23rd from some pet food web-site.

    Found out when I got to Kroger and card would not work. So canceled card, went through all the getting new one and reporting the ID thief and resetting passwords now everywhere.

    US government card, so they are doing an investigation on all that. If I get the money back, who knows. Annoying hassle is the worst part of it all.

    On the form they ask how one wants the payment. Kind of leary of routing it to my bank, gezz. Says if you want a check click the link, but the link don't work. Filled in all the info and it won't take. Says errors on form. I'm like where? It copies everything right from your Facebook info?

    My friend ordered a product from facebook and the very next day her bank account was hacked. They got away with $20,000 dollars. I would never order anything from facebook. Way too risky.

    I don't trust giving out personal information online.
    I would never give out my personal bank account information online either.
    For peace of mind and security reasons, I have opted to fill out the physical paper form, and requested a physical paper check.
    I even had concerns about doing this because of the possibility of identity the theft.
