Not arguing Susan but they are not all self reported. You'd have to sift through the data to find out how many are self reported vs. reported by health care provider. Of course, I don't know how deaths could self report. :)

Another thing about reporting, it's only anecdotal of course but most people don't complain, especially if the process is cumbersome so It's likely that the adverse reactions are significantly under reported.

Well, that doesn't change anything. The patient describes the effect to a healthcare worker who reports it. My only point is that these are not necessarily side effects of the vaccine, but rather symptoms noticed after the shots. That's why I mentioned psychosomatic. For example, I have a sore neck, but I'm really not paying much attention to it. I go in and have the shot, the next day I get an alert on my phone to report how I feel after my shot the day before. I enter, "my neck is sore" and hit enter. Now, the sore neck has NOTHING to do with the vaccine, but it's how I was feeling at that moment.

I will get my second shot this week. I see the vaccines as God's gift to us. Thank you Lord!

Yeah, it really isn't that easy to report, hitting a button on your text. The process is a bit more cumbersome which is why I said that reactions are most likely under reported. If the process was as friendly as you make it out to be I'm sure the #'s would be much higher.

What will I need to fill out the report?

Patient information (age, date of birth, sex)
    Vaccine information (brand name, dosage)
    Date, time, and location administered
Date and time when adverse event(s) started
Symptoms and outcome of the adverse event(s)
Medical tests and laboratory results (if applicable)
    Physician's contact
VAERS - Report an Adverse Event

To vax or not to vax?

I believe that I Corinthians 8 and Romans 14 is the wisdom and will of God (at least for me) in such matters.

HE is to be our focus and our sustainer and our faith must ultimately be in Him.

This I say therefore, and testify in the Lord, that ye henceforth walk not as other Gentiles walk, in the vanity (futility) of their mind, having the understanding darkened...
    (Ephesians 4:17-18)

Blessed is the man that walketh not in the counsel of the ungodly...
    (Psalm 1)

VAERS - Report an Adverse Event
The process I described was literally what I did after my shot. When I went to the waiting area, there was a QR code on the wall. From there, I signed up, provided my phone number, and they texted me daily with a link to a form to report my symptoms.

Unless it was the link I posted, it wouldn't make it into VAERS. If it was a link to VAERS, you still had a ways to go..

VAERS - Report an Adverse Event

    The form ..
